Policy Stools will be examined and a description of them recorded using the standard Bristol Stool Chart for guidance.
Examination and recording of stool types will take place within the Compassionate Care Team Ltd Care Planning system.
A blanket approach will be avoided, and the use of the Bristol stool chart determined on an individual need by need basis.
Procedure There are a number of reasons why stool categorisation will be needed and an assessment will usually have taken place, for example, a care needs assessment followed by a continence maintenance Care Plan.
The decision to use the Bristol Stool Chart should be discussed with the Service User. This discussion can then provide an opportunity to look at promoting the Service User’s health and offering a management plan based on the results of the stool assessment.
Compassionate Care Team Ltd will ensure that any monitoring of stools is documented in the Care Plan and that Care Workers understand how to complete the stool monitoring form. The Care Plan review date will be documented and any changes in care support will be recorded and reported.
The template Bristol Stool Scale Chart attached to this policy can be used to categorise the stools when observed.
Staff will record the categorisation type number in the BO column of the Activities of Daily Living (ADL) recording sheet.
Staff will review the information during the normal Care Plan review, or outside of the scheduled review time if assessed as required, as set out in the Care Planning Policy and Procedure.
Staff responsible for the completion of the Bristol Stool Chart will be trained to understand the different types of stool, where and how to document results, as well as the action to take in the event of abnormalities and concerns.
